VAHA 2009-2010 Regular Season Hockey Registration Sunday, September 20th from 3- 6 Wednesday, September 23rd from 6- 8 Please come to registration with ALL FORMS COMPLETELY SIGNED and FILLED OUT. They include: 1. Registration Form - top must be completed only 2. Waiver Form - must be signed by both parent and player 3. Volunteer Form - must be signed 4. Consent to Treat - must be filled out completely 5. Player Code of Conduct - must be signed 6. Parent/Guardian Code of Conduct - must be signed 7. IMPORTANT!! You must register for USA Hockey Insurance, which expired August 31, 2009. Log onto www.usahockey.com and complete registration. The cost is $40 and is excluded from your registration fee. * YOU MUST PRINT THE CONFIRM PAGE and BRING TO REGISTRATION * VAHA is asking that at least ½ of the fee be paid at time of registration. If you are unable to pay ½ at registration, please contact VAHA to make payment arrangements. ** There will be a $20 late fee for any registration received after Sept. 23rd for any returning hockey players.
